o The consultation time provided is indicative and actual consultation time may vary.
o The appointment slot will be released in case of late arrival of the patient and the patient will be treated as a walk-in patient and consultation should be subject to availability of time with the respective doctor.
o Complete details of ‘New’ patient should be filled accurately and we may also use this information for data analysis and marketing purposes. By accepting these Terms, you expressly permit SGRH to use the data provided by you for the said purpose.
o We track the Internet address of the domains from which people visit us for trend analysis.
o We may use the Internet address to identify any fraudulent transaction.
o If you do not wish to provide us with the required information, kindly click on exit button to exit the appointment booking application.
• SGRH shall not be responsible for any such failed transaction, where the bank account of the user of the WebSite is debited for the transaction amount and the transaction is not completed, for any reason whatsoever. • You agree and undertake to indemnify and keep indemnified SGRH, the concerned doctor/physician and SGRH Hospitals for any losses, costs, charges and expenses including reasonable attorney fees that the concerned doctor/physician and SGRH may suffer on account of (a)misdiagnosis / faulty judgment /interpretation errors / perception error arising from (i)your failure to provide correct and / or complete clinical information / history about the patient in timely and clinically appropriate manner; or (ii)suppression of material facts; or your failure to provide relevant clinical information about the patient; or (iv) misinterpretation of the advice / prescription / diagnosis by you; or (v) failure to follow doctor’s advice / prescription by you; or (b) incorrect or inaccurate credit / debit card/Bank Account/UPI details provided by you; or (c) using a credit / debit card/UPI/ Bank Account which is not lawfully owned by you; or (d) if you permit a third party to use your password or other means to access your account
